Due to the sensitivity of record-level information, thoughtful consideration must be given before this type of information is disclosed. To assist those looking to request record-level information, the following document provides an overview of the request process.

Requesting Record-Level or Identifying Information – An Overview

In order to properly consider all the aspects of a request for record-level information, the Centre requires all such requests to be submitted using one of the following applications.

• Application to Request Record-Level or Identifying Information – Research
• Application to Request Record-Level or Identifying Information – Quality Assurance
• Application to Request Record-Level or Identifying Information – General
• Application to Request Record-Level or Identifying Information – Additional Information
